This adorable Father’s Day printable questionnaire for kids to fill out for dad is such a special gift. It doesn’t take much to make dad feel super loved on Father’s Day, just the fun and sweet words from their kiddos.

To accompany last month’s Mother’s Day printable, today I’m sharing the Father’s Day version!

This Me and My Dad questionnaire is a super easy, incredibly sweet way to show dad some love.

And who doesn’t want to show dad love?  Take the time to really talk with your kids about the things they enjoy doing with dad.

It’s a fun opportunity to help them remember things that dad teaches them. I’m sure you’ll get a few laughs, too!

We did this printable every year and it was always dads favorite. Some of the answers just end up so silly.

My kids have used this Father’s Day printable every year and the answers are just adorable. “My favorite thing to do with my dad is: everything!” Isn’t that just precious?

All you have to do is click that link and hit print!  Then, you can help your little ones fill out the questionnaire or let the older ones do it solo.

I labeled it with the year in case you want to make one every year.  It would be fun to keep these together or put them in a scrapbook.

    Hello! I used this with my kids last year and it was a big hit! I was hoping to use it again each year, however, this has the year 2014 printed on it. Any chance you could make one exactly like this and leave the year open to be written in so that it could be used year after year? I really think it would be fun to be able to see how their answers change year to year. Thanks!

    Thank you for always updated the Father’s Day printable. We started using this a few years ago and it’s not tradition for my kiddos to each fill out a page for my husband. I’m a little late this year in true busy mom fashion but the cute thing was that my husband remembered and requested it! Thanks again!
